I prefer the "illusion as hologram" version. The illusionist creates an incredibly real-looking, 3D model, complete with sounds and smells at higher levels. Everyone can see this hologram (yes, even undead <sound of can of worms being opened>). If you make the save, you realize it is a hologram, but...

I voted to remove them. I have no problem with arcane healing. I have no problem with C&C being different from D&D. I just can't accept the idea of "illusion" being real. Hmmm. Maybe I've been missing something, but I've never used a roll for surprise. We've always just gone with the situation--it's either obvious who is surprised, or if it's not, then no one is. Works well, and is simple. (and no wisdom "surprise-busting" problem, either)

I like alignments for role-playing purposes (as a guide, not a straitjacket). However, the only characters with detectable alignments in my game are clerics (and paladins) of good or evil deities, undead, and residents of the aligned outer planes (hells, abyss, heaven, etc.)

It's possible, but in the end I found it to be too much trouble--there are so many thing different about the LA creatures, that you can't easily substitute them. And it is harder to convert NPCs. But it can be done.
