What worked for me was always, roll initiative, and if the PCs win, ask them what they are doing. Then the NPCs react. or visa versa, as in, roll initiaive, PCs lose, Hey, this guy hits you, this guy misses you, this happens... etc etc... if you lose initiative, youre on the defensive, so planning/...
