I am currently running my group in the World of Greyhawk, using the 1983 boxed set (set in 576 common year). I get the benefit of an established setting, but allowing me to fill in all the gaps as I see fit.

My bad, I re-read the 1E PHB (page 105) and saw the entry of 0hp=death and did not think to check the DMG.

My recollection of playing in middle-school was that 0 = unconscious, and -1 = dead. But then that was 26 years ago, too.
