Yes to both questions. Detect alignment requires the chosen alignment (law, chaos, good, or evil) be declared when cast. In the case of the paladin ability, the choice has already been made.

The disease section is new in the CKG, and the tables do appear to have some issues, notably the use of "Affect" when it should be "Effect". The triple asterisk should indeed apply to 8 and 9 only in those tables.

The sentence in question probably should read "All beings have a minimum SR of 1", rather than the current wording or even the wording I used in the PHB.
