ISO: A certain character sheet

OK, this is silly, I know. But I downloaded a C&C character sheet from SOMEWHERE a while back. I printed out 6-7 copies or so, but now I can't find the file I printed from. I know there are lots of options for character sheets out there, but Iwas hoping to find this one again.

I was one page (front only), has a small C&C logo in the top right corner, and box in the lower left corner for spells. Also, there's a line in the bottom right corner for a Last Will & Testament. Any ideas? (I wish I had a working scanner, then this would be irrelevant.)
