Question about the Writer's Guidelines

There is a 'Disclosure Form' mentioned in the Writer's Guidelines ( http://trolllord.com/downloads/index.html#tlgdocs ), saying "a copy of this form appears below". Unfortunately I do not see it in that document.

In the resources section there's a NonDisclosure Agreement Form (NDA). Is that the Form mentioned in the Writer's Guidelines or is it something else? If it's something else, where do I find it?

In the Writer's Guidelines it also says that for a proposal via e-mail one "should include a completed form in electronic format that is attached to the e-mail"

If it refers to the NDA then there is a section that must be signed. Is the scan of a copy signed by hand required for the proposal or can one just type one's name via Word at that stage? (The final document, if asked for by TLG, must be sent with a signed form of course, my question is only for the e-mail proposal)

Those guidelines are very old, and probably have not been updated in years. When the site was ported over to a new server last year, all of those old links were probaby broken.
