Slow cuts movement and attacks by 1/2.
How do you folks adjudicate its impact on those with only one attack? 1 every 2 rounds or leave at 1 per round? Use the d20 version?

I'd go with 1 every other round based on the wording.

I do the following:
Opponent gets -4 AC; moves at half speed, and attacks at 1/2 normal rate. If 1/2 normal rate is not possible (because they get 1 attack), the opponent is "slow" like a zombie: they always attack in combat last. There is no impact on spellcasting times.
However, in my games, (most) characters may also gain additional attacks more rapidly than standard so the period where they have the alternate effect is somewhat slim. Wizards never get added attacks without magic, either spell or item, but they're supposed to be hurling magic anyway so who cares.

In my games, additional attacks are also gained a bit faster then they are in base C&C as well, but when the Slow spell is put on a creature with one attack, they get that one attack ever other round.
