Customized C&C Books?

Is it possible to order customized C&C books? Like say, one binding with another interior?

I.e., the 5th printing cover with the black and white 4th printing interior?

Not that I have heard of. As far as I know, the Trolls don't print their own books anymore, at least not when they are hard cover. They have made customization offers in the past, but only specific ones, for a limited time.
Since its 20,000 I suggest "Captain Nemo" as his title. Beyond the obvious connection, he is one who sails on his own terms and ignores those he doesn't agree with...confident in his journey and goals.

I don't remember what year it was, but they were allowing people (for whatever the price was) to have their house rules added into the phb. Generally, the hardcovers go to Wallsworth Publishing in Marceline, Missouri. The softcover material is printed at Troll HQ. There was a brief time when the boys did their own hardcovers. I believe it was the intial runs of the CKG and M&T of A, maybe even OG&M.

Yeah, I decided to give in and order the 5th printing through Noble Knight Games via ebay. The colour interior will be o.k. I'm sure. To be honest, Peter Bradley is perhaps my favorite fantasy artist, and the 5th printing cover totally doth it for me.

I don't customize my main rulebooks but I do produce a new Campaign Handbook for every campaign I Dungeon Master. My handbook for my current C&C Forgotten Realms campaign clocked in at 34 pages. I put in them any customized rules I am going to use plus background information on the campaign, campaign world, and several maps.

Depends. Usually I print with a business class color laser printer on higher quality heavier and brighter paper duplex-ed. I lay the booklets out just as you would if you were to have them printed at a service and produce a PDF file for the locals that dig tablets. Usually I print them myself and put them into a two-pocket binder so that the player's can keep their notes and character sheets in them. A few times I went deluxe and used view binders with the round rings but this costs a lot more since I produce one for every player. At-least twice my group went the super deluxe route and we had a service print our campaign guide. This was back during the heyday of v3.5 and we were using a home brew world with a lot of funky alternative rules and the page count was just over 100 pages for those but that is pretty largish for them and uncommon. We also used a lot of local original artwork in those which is also uncommon for us.

During 4E I went full color and mimicked the layout of the 4E books. For Castles & Crusades so far I have been doing them black and white and mimicked the layout of the first edition Player's Handbook as far as style and fonts. It is very retrograde looking and nostalgic but it actually shows how poorly those books (in some ways) were organized. :)
