New poster here. I saw the Combat Dominance (Revised) thread and liked it, along with Serleran's addendum regarding increasing the hit die type. Here is my revision of that revision. Deconstruct at will and let me know what you think.
Combat Dominance (Re-Revised)
At 2nd level, the fighter gains an extra attack when scoring a hit on an opponent with half the fighter's hit dice or less. The extra attack should be considered a 'follow-through' where the fighter is exploiting a weakness in an opponent. This extra attack may be directed to a different opponent as long as they are within reach and meet the hit die criteria. Thereafter, every four levels the affected hit die type increases to the next die (d4, d6, d8, d10 and d12). This ability CAN be combined with the Extra Attack ability half the character's level per day.

I actually like that one. Simple, easy to keep track of, and much more likely to stay relevant.

I'm glad you liked it... I've been using it quite successfully for some time now. However, I would re-word the first sentence to:
'At 2nd level, the fighter gains an extra attack when scoring a hit on an opponent with half the fighter's hit dice type or less.'
Half the fighter's half dice at 4th level for example would mean he could get an extra attack on all creatures which are 2HD or less regardless of the hit die type. It's just a suggestion (and in this case a single word) but changes nothing to your intention.
As to the other changes you made, you chose to improve the hit die type instead of increasing the possible number of attacks but allowed for the Extra Attack ability to be combined with this one. This works just as well!
The one question though is regarding ranged attacks. Since intended as a follow through, you may consider putting that restriction back in (as in Combat Dominance Re-revised can only be used with melee attacks). Otherwise, you may end up with a devastating high level archer!
This is one of the great things about these forums and the attitude of the game... an exchange of or building upon ideas by the community!

I did unintentionally leave out the last sentence. "This ability is only usable with melee weapons and cannot be applied to ranged combat.".
As to the limitation of half the fighters hit die type, I'll have to examine that modification.
