Here are some items which I converted to C&C. They are specific to the Dragonlance setting, but I guess they can be used in other settings, too, by changing the background. Enjoy!
Armor and Shields
Dragonarmor
This type of armor has been used by the forces of the Queen of Darkness throughout the many ages of Krynn. It consists of a padded tunic and leggings with a scale mail, breast and shoulder plates. The helm is divided in two pieces and it protects both the back of the neck and the face. The armor is designed to allow freedom of movement, protection and warmth while riding dragons at high altitudes.
Dragonarmor is considered +2 scale mail. It is custom fitted, and when worn its EV is 2. Those who wear the padded tunic and leggings subtract 1d6 points from any cold based attacks or damage. The helm is considered a great helm.
If someone who is not the owner tries to don the armor, he will find that it fits too loosely or too tightly, and will suffer a -1 penalty on all physical actions (including combat); its worn EV is 3.
Helm of Griffon Mane
This helm took its name from hero Flint Fireforge, who, having always claimed he was allergic to horses, insisted to his dying bed that the helm's mane must be from a griffon's mane.
This helm of dwarven make is decorated with a horse's mane, and it will only fit a dwarf, granting +1 bonus to AC.
The Plate of Solamnus
These suits were the finest in all the lands, and they were created when the Knighthood was formed. They take their name from the first ever built, which was for Vinas Solamnus, the founder of the Knighthood.
A suit consists of breastplate, shoulder guards, arms fittings, leggings and a normal helm. All the suits are beautifully tooled with the markings of all three Solamnic orders, however the Order of the Rose is prominent on the breastplate.
The armor is the equivalent of +5 plate mail, but only for characters of good alignment. For characters of neutral alignment (lawful, true or chaotic), the bonus reduces to 0, while for evil characters it is -5. Furthermore, evil or neutral characters suffer 1d10 additional points of damage from any damage received when wearing the armor.
Shield of Huma
In many tales it was said that the shield of Huma never failed him and that it often saved his life during battles against dragons before the dragonlance was employed. It is carved with intricate symbols of the Knights of the Crown.
The shield of Huma is a +3 medium shield, +5 vs. dragons. Furthermore it protects its wielder against dragon breath weapon attacks so that a successful saving throw results in no damage, and a failed one in half damage.
Solamnic Armor
The traditional armor of the Knights of Solamnia is granted to a knight when he attains the title of Lord. It consists of breastplate, shoulder plating, normal helm, shield and spurs.
Solamnic Armor is equivalent to +1 plate mail with +1 shield.
Weapons
Dagger of Magius
This dagger is said to have been created by the famous archmage, Magius. Later he came into the possession of Raistlin Majere, who purchased it from the Tower of High Sorcery.
The dagges is six inches long, carved int the shape of a dragon with the tail as the blade.
The dagges is +3 to hit and damage, and has the ability to remain undetected in a search of the mage's person.
Dragonlance
Dragonlances are artifacts created at the end of the Third Dragon War and used to defeat evil dragons. There are two kinds of dragonlances: footman's and mounted. Each type is made of the same silvery metal (called dragon metal) that gives off a healthy silver glow. How well the lance was made and under what conditions determines how the lance performs in battle. The ideal conditions require that it be forged by someone wearing the Silver Arm of Ergoth and the Hammer of Kharas (q.v.). A lance forged with both artifacts receives a +4 bonus to hit and damage, while a lance made with only one of the artifacts enjoys only a +2 bonus. These bonuses are in addition to any other bonuses listed for the weapons.
Footman's Dragonlance
This lance is 8 feet long, and it can be used as a hand-held weapon, or thrown with some difficulty. It has the same characteristics of a light lance, but if used against a dragon it inflicts damage equal to the current hit points of the wielder. The lance is +1 to hit, unless it is thrown, in which case it is -2 to hit.
Mounted Dragonlance
This lance is 16 feet long and is most often wielded while mounted on a dragon. It is buttressed by a harness mount that enables swift turning. The lance is ineffective on mounts smaller than dragons due to its bulk. It acts as a normal heavy lance if not mounted on a dragon. It has the same characteristics of a heavy lance, but if used against a dragon it inflicts damage equal to the sum of the current hit points of the wielder and her dragon mount. The lance is +2 to hit when mounted on a dragon.
Frostreaver
A frostreaver is a battle axe made out of extremely dense ice gathered from a secret location on Icewall Glacier. The knowledge to build these weapons is held only by the Revered Clerics of the Ice Folk, and it includes use of thanoi oil and other ingredients. The cleric must work for a full month, and even then there is only a 33% chance to have a working axe. Due to its nature, one day of above-freezing temperatures will cause a frostreaver to become useless as a weapon. In a warm environment the decay requires 1d6 hours.
A frostreaver is the equivalent of a +4 battle axe. Due to its bulk, it can be effectively wielded only by characters with Strength 13 or greater.
HAMMER OF KHARAS
The Hammer of Kharas is a mighty artifact
and is the only hammer that can forge a true
Dragonlance.
Its name comes from the great dwarven hero Kharas who fought in the Dwarfgate Wars in the
first years after the Cataclysm.
The hammer appears to be a war hammer
+3, of twice normal size and EV. It cannot be lifted by
a character with a Strength of less that 10, and
anyone without strength prime of 17 or more suffers
a -2 penalty to hit with it.
The hammer acts as a hammer of disruption
against undead and creatures from the Abyss.
It turns undead as a 10th-level cleric.
The artifact has Will 17. The hammers motivations are to preserve
the security of the dwarven race and to
further the cause of good.
The Hammer of Kharas has the following
special abilities, at the 20th level of magic use.
- Detects evil as a paladin
- Gives wielder immunity to fear, both normal and magical
- Cannot be affected by magic less than 5th level
- Casts prayer once per day
- Provides protection from arrows once per day
- Acts as a potion of giant strength (Str 22) once per day
- Cure serious wounds once per day
- Inspires magical awe in all dwarves and derro: upon seeing the hammer, they will be stunned into inaction, being only able to defend themselves (no saving throw). The effect lasts as long as the hammer within sight.
The hammer chooses when to activate any
of its abilities.
Mantooth
This long sword is also known as Spellcleaver, Darkstar and Magefool. Its history is unknown, although it has been among the Silvanesti elves for untold ages.
Mantooth is a +1 long sword, +2 vs. magic-using and enchanted creatures (which includes magic-users, spellcasting monsters, and created, summoned or gated creatures).
It is able to sever magical barriers of any kind, with a cumulative chance of 1% for each damage point inflicted (e.g. 4 points of damage translate to a 1+2+3+4=10% chance). The sword can also turn a spell against the caster, if the wielder of the sword makes a successful to-hit roll vs. AC 20 (only the BtH is considered).
Nightbringer
This weapon was forged by Black Robes in the Tower of High Sorcery in Ergoth to fight against the Church of Istar.
Nightbringer is a +3 heavy mace, with the power to blind for 2d6 turns when it strikes, upon the utterance of a command word (CL 9 save vs. spells to avoid). If a character of good alignment tries to pick up the mace, he must roll a CL 11 save vs. spells or be blinded permanently; a remove blindness spell can be used to remove the condition.
Rabbitslayer
This dagger belonged to the Kender hero Tasslehoff Burrfoot, who claimed he discovered it in the ruins of the sunken city of Xak Tsaroth. He called it Rabbitslayer since Caramon Majere told him that it would have been useful only if he was attacked by ferocious rabbits.
Actually, the dagger was blessed by clerics of the ancient days, and it is a quite powerful weapon. It is a +4 dagger, and it always returns to its owner within 1d20 hours after it is lost or stolen. It can pass to another person only if its owner gives it away of his own volition.
SWORD OF HUMA
Like the Shield of Huma, the sword never failed him and he used it during battles against dragons before the dragonlance was employed.
This sword is as large as a bastard sword, although it is a +2/+4 long sword, Dragon Slayer (but it doubles its damage against all chromatic dragons).
Wyrmsbane
This sword was created during the Third Dragon War and was then lost later in the Age of Might.
Wyrmsbane is a +2 two-handed sword, bane vs. dragons and draconians. However, its bane bonus vs. black and sea dragons is 4d6 instead of 2d6. The sword does not become stuck into a dead baaz draconian. Wyrmsbane can also locate objects in a 45' radius three times per day. If the owner of the sword desires to find something that he knows well, the sword will steer him in the right direction.
Wyrmslayer
This mighty blade was the favored weapon of the ancient elven hero, Kith-Kanan. It was forged in Silvanesti during the Second Dragon War and remained in the royal house until Kith-Kanan led his people to Qualinesti. He is said to have been buried with the sword.
Wyrmslayer is a +3 two-handed sword, bane vs. dragons and draconians. The sword does not become stuck into a dead baaz draconian. A character holding the sword by the hilt gains a +3 bonus to all saves against dragon breath attacks, and spells cast by dragons or draconians.
Wyrmslayer has the disconcerting tendency to buzz aggressively whenever it is brought within 30' of a true dragon (not a draconian). This buzzing is loud enough to be heard clearly and always awakens a sleeping dragon.

Nice write ups! Plus the way you wrote them up touches on the tidbits of history that have always intrigued me about DL.

Since its 20,000 I suggest "Captain Nemo" as his title. Beyond the obvious connection, he is one who sails on his own terms and ignores those he doesn't agree with...confident in his journey and goals.

I'm not a huge DL fan but these are GOOD write ups.
