1. When is a character's level supposed to be added into their roll for an attribute check? If I recall correctly, the PHB's examples sometimes have it added in, and sometimes don't. For example, the PHB talks about an attribute check for a fighter pushing over a boulder onto an opponent and says that the fighter's level should be added in. But I'm pretty sure there are other examples given of attribute checks where the character's level is ignored. Am I missing something?
2. At 3rd level, monks gain the ki strike ability to strike monsters as if their hands were +1 weapons (the bonus goes up as the character attains higher levels). Does this merely mean that the monk has the ability to hit creatures that can only be hit by magical weapons, or does the monk also get the corresponding bonus to his/her to-hit and damage rolls? I.e., does a 3rd level monk get a +1 bonus to hit and a +1 bonus to damage for unarmed attacks?

A character doesnt add their level when emulating an ability of another class.

By my understanding and in my campaign, they do get a +1 to hit and damage when the monk is at the level to do so. It also gives an attractive reason to play a monk (all things considered).
I remember in older editions of AD&D, you sometimes had a enchanted weapon requirement of '+x' to hit certain creatures but I haven't used or come across a critter in C&C that had that anything above a '+1' requirement. Are there any creatures that require a '+2' or higher to hit in the M&T?

Yes, through errata there are, like golems and all elementals, and some forms of undead. "DR" was removed and it was decided that monster text descriptions would be used for such things, much like the OAD&D MM. I would not give the monk the +s to hit and damage, since they already do considerable damage (and get multiple attacks) and have a pretty damn good BtH as well. The one thing that refrains them from being better than the fighter at fighting is that they don't get bonus after bonus after bonus. Besides, they can use magical cesti.
