What is it?

Hmm... with one character I played, I don't even know where to begin.
I suppose constantly using other party members (two of which were family) as leverage for more money and power by those who would have me kill them.
No specific incident though... there were just too many with the one character.

Oh, and to continue the story...
This was set in Al-Qadim, which has rules for Calling on Fate. In dire circumstances, you can Call Upon Fate to give you a fighting chance for survival. However, there is a 1% chance that Fate turns against you.
Well, this ex-paladin was surrounded by multiple scary undead. He Called Upon Fate, and sure enough, 00. I had a roc swoop in, pick him up, fly to about 20,000 feet, then drop him over the ocean.
Oh wait, that was me the DM being evil... heh...

Well I played a neutral evil wizard in an evil party, so he did some bad stuff. Probably the worst was to threaten prisoners with the anti-paladin. He say "I need to know what you know. I know a little already, so if you contradict that, I'll know you're lying. If you cooperate, you die quick, if I don't think you were helpful, I give you to him" pointing at the psycho Anti-Pal.
Then, when I was done with them, I gave them to the AP anyway. He'd torture them to death as a sacrifice to his gods.
Terrex was not a nice person.

One of my first characters, a Gnomish illusionist/thief who was tired of being pushed around by all the "bigs" somehow forgot to mention that the spiked pit trap that he found *triggered by weight which makes it even funnier*. Needless to say, the party died messily and the heartbroken Gnome had to inform our employer that he would need a bit more help checking out that ruin.

I played a thief in an AD&D campaign, he wasn't the nicest of fellows.
He used the party as bargaining chips to get something he was after from a high level enemy mage. The bargain was accepted and he led them into an ambush.
He didn't have a change of heart at the end or see the error of his ways. The party was killed, he even backstabbed the fighter during the ambush, collected his reward and went on his way.

A bard in the party struck up a relationship with a lovely, lady fighter (and new recruit into the group). The couple's development was great roleplaying on both player's parts. Eventually the two character's were engaged to be married, but the wedding met delay after delay, since although the bard loved his "lady", he couldn't take the final leap from bachelorhood.
A year or two of real time passed as we continued the campaign. Party members died, or left, or grew in power. The bard became affected at one point by an insanity-inducing magical item, but none of the other party members were aware of the change. Slowly the bard's mind became unhinged, with his way of thinking becoming more and more evil. Even his lady love was unaware, and soon found herself carrying his child.
Secretly, the bard allied himself with a drow priestess the party had encountered briefly during a foray into the Underdark. In order to prove his loyalty to her and to help facilitate some sort of arcane ritual, the bard agreed to provide a sacrifice...
One night, a commotion was heard within the temple (and group HQ) constructed by the party's chief cleric, screams and other such sounds emanating from the expectant mother's bedchambers. When a contingent of the temple guard made it through a (not supposed to be) magically barred door, they found the fiance gutted, parts of her strewn about the room, and the child harvested... The bard was seen stepping through a shadow, with a laugh on his lips and a bloody bundle in his arms as he gated away.
The party cleric was able to somehow save the woman, to bring her back from death. The rest of the party joined her in the quest for the (presumed alive) child and vengeance on the bard and his dark allies.
We took a gaming break for a while after that one.

Other then killing the occasional party member, or "setting up" the rest of the party for being killed or robbed of all of their stuff, I almost exclusively stay away from evil acts. I just don't have it in my persona to play a character that evil. Or to play a believable character who does evil stuff.
I guess that makes me a bad roleplayer, IDK. Just never cared for it to be honest.

Too many things, from having a cleric act as a midwife and casting "bestow curse" on deliverance of babies, to acts of cannibalism (animating the dead to make them cook themselves, since she was too lazy to do it) and other sordid things... really, quite a lot. This was with the same character. Evil clerics rock.

Understood. Pretty awful scenario.

Maybe, but what a powerful reason to continue adventuring! Sounds like to me, that regardles of the disgusting nature of the situation the party a suprememly large motive for continuing the adventure.

Yeah, the whole scenario leading up to what happened took over a year of gaming, what with side adventures and so on. After it happened and our break completed, we returned to the gaming table. Over 6 months of gaming later the remainder of the party had brought the bard to "justice", and toppled those behind his insanity and actions (the drow mentioned earlier). Though nearly killed by the woman he'd wronged, the bard was cured of his insanity, then the player worked for a long while afterward to atone for his crimes.
His former "lady" never forgave him, but she did let him live, feeling it was more of a punishment than a quick blade, after his sanity (and morality) returned.

I was DMing a group for over a year. The party would dice for magic items. One player, a cleric, almost always rolled high, another player, a fighter, usually rolled low. One day the player with the fighter PC tells me he is going to hire an assassin to help him kill the cleric PC and his henchman. He spends all of his money to hire the highest level assassin he can get. He has the assassin attack the cleric PC, and he goes after the henchman. His plan works. He robs the cleric and runs. What he doesn't do is dispose of the bodies, so another PC, a ranger, pays to get the pair raised from the dead, and together they hunt down the fighter, kill him, get their stuff back, and destroy the body.

As a player, I was playing a fighter PC. There were two other PCs, both fighters. I had reached 4th level, and had a magic-user henchman. After a trip to a dungeon, we were heading back to town. The other two PCs were pretty beat up, but I was almost at full health, and my magic-user still had spells. My PC had been evil from the beginning, and I'd been planning on robbing these two for a while, and now I saw my chance. My magic-user casts sleep, and one of the two goes down. The other fighter proved to be more of a challange than I had expected, and we were fighting for several rounds, and I was close to getting beat. My henchman casts a spell and opens a gate and a demon starts coming through. Seeing this the other PC flees, and I let him go. The gate and the demon were illusionary. I rob the one fighter and leave him tied up and nude in the woods. The other fighter ends up finding him, and they both swear to track down my PC and kill him, but they both ended up getting killed on another adventure before they ever got the chance.

This may be better classified under 'stupedist things the party has done". During a CoC game, the GM privately provided one of the players with a 'tip' as to where the evil cultist were located. The player didn't write the house number down, and later that evening when it was time to go stop the cultist he got the address wrong. Our whole group went to the house NEXT to the cultists, and proceeded to torture and slaughter the people living there (hey, we THOUGHT they were evil cultist..). Once we were finished, the cult leader came over from next door and asked if we wanted to join up. Not our finest hour...

GM (aka me): "As you stand over the lifeless body of the evil merchant who you found out had stolen the magic ring, you see his young 8-year old daughter standing in the doorframe. She is absolutely terrified at the sight of you standing over her dead father with a bloody short sword in hand."
PC Thief: "I pull out my crossbow."
GM:"What? This is an 8 year old innocent girl!"
PC:"She's going to alert the guards! *rolls dice* Yes Maximum Damage!"
I start bashing head on table.
PC:"Does she have any loot on her?"
Volume of head bashing increases.

This is not really "evil," but it is the biggest disaster that happened in my games (and was entirely my fault). I was very new to DMing, so I didn't know any better. I am definitely never gonna let this happen again, to be sure.
Anyway, it started off with a boss battle at the end of a dungeon. The heroes had been tracking down an evil cult of Hextor that was searching for an ancient sword with the power to tear holes in the fabric of the plane. A deadly weapon, to be sure, but the sword wasn't really the problem. Well, they made it to the end of the dungeon and had to fight off a group of clerics of Hextor; they managed to kill the lead cleric, so the surviving enemies retreated, using their magic rings to teleport out.
Afterwards, one of the players had the bright idea of figuring, "The guy we killed must have one of those rings, and I want it." I don't know how I failed to see that one coming, but next thing I knew, our fighter was running around with a ring of unlimited teleporting. He would use it to teleport into the center of an enemy base, grab a guy, then teleport him to the middle of nowhere and leave him. I didn't fully understand all the rules of the game at the point, which made the situation worse in favor of the fighter. But man, that ring really messed things up for me.

I have a new one:
The group running the game was charged with going into a cave and saving a small gang of adventurous younglings that have gotten in over their head. Well they found the children well enough and rescued them. Having no more healing left the Oaf in my group and the dwarf (this is LA btw) had to carry 2 of the small boys.
Well on the way out thy group got ambushed. The able small boys where ordered to flee while the group held the beasties off. The oaf and the dwarf sat down the hurt kids in order to enter the fray. Well the group ended up fleeing, but left the boys there!!
They ultimately became bait for the hungry beasts that saw it as a free meal and were easily dispatched of as the enchanter snuck back into the cave and blasted them with a spell.
Now whether the boys where left there on purpose or not is yet to be seen, but either way it was pretty horrific thing to do, and actually a little heart wrenching especially as the group lied to the parents of the boys about what happened.
Needless to say the oaf (who is my avatar while run the game) was very upset, and admits that he was too dull witted to realize at the time he left them there, following orders from Fester the Dwarf. (The Oaf has had a bad past with joining up with a gang of theives which ended in an accidental killing of a small girl, which gives him a sort of backwards and confused crusade to help and protect people.)
Needless to say the group didn't receive full merits for the rescue.

In an original Dragonlance campaign I DM'd, the guy who played Gilthanas killed his older brother, so he'd be next in line for the throne of the Qualanesti elves. Then he killed his old man. Then he sent his elite guard on death squad missions, to kill communities of gnomes, so as to slowly change their alignment to evil. Meanwhile, he salted the fields of the Thorbadin dwarves and locked them in the mountain in an attempted genocide. Later, he killed Raistlin.
This same guy, in another campaign, as an evil cleric, would cast telepathy on his victim and then bash their skulls in with a mace, so he could experience what it felt like to have your skull caved in. This was a long time ago, and we were all very young.
