Could you explain why BtH may, or simply are not, added in when factoring things like Pummeling, grappling, etc...

It is, for monk characters: "For example, characters who are not of the monk class who wish to punch or kick are attempting a pummeling attack." That very line implies the normal rules apply, and monks always get BtH for every type of unarmed combat maneuver - that is part of their archetype after all. This is a residual concept from level benefits are added only to those actions which would qualify for them, so, unless the CK decides Joe Blow the Fighter should be well versed in the arts of weaponless combat (probably very likely, but not so much for a rogue or wizard.)
