Lance Attacks
How do they work exactly? Can you continue to charge with the same lance every round? I'm thinking the lance might break or get stuck in the opponent. Does the opponent get a free attack as the knight charges past?

Assume that, by the book, the lance doesn't break. Weapons don't break and armor doesn't lose effectiveness, by the book.
If breakage is something you wish to introduce as a factor, make a saving throw for the lance (item save) after a charging attack. You might also consider breakage as a possibility on a roll of natural 1 for the attack (or 1-4, or whatever range pleases you).
As for getting stuck, I can see that as a possibility. By the book, the issue never happens with spears and polearms in melee, or with lances on horseback. It's a level of detail that is overlooked in order to make the game move more quickly. I suggest a house rule if you desire this level of detail. Consider forcing the lance-wielder to make a strength check to maintain a grip on the lance and pull it free.
The opponent does not get a free attack for a charging opponent on horseback riding past. This is especially the case if the lance attack successfully hits that opponent. Exception: if the opponent is also using a longer reach weapon (such as an awl pike) and is anticipating the charge, the opponent may attack before the lance-wielder.
You may wish to allow an opponent missed by the lance to make some sort of attribute check or checks to determine whether this happens in your game.

Terrain is a consideration. You must be able to charge, which means a straight line, and up to what, double speed, to pull it off. Charging on a horse means basically flat land. Add some rocks, some tree stumps... goodbye lance.
